Outbreak of Salmonella Newport

Whooping cough cases increase

Patients are free join any practice at Minden they wish. If no preference is expressed, then practices take it in turns to accept new patients on a monthly basis and you will be assigned appropriately. Please ask at our reception for a registration form. All practices currently have open lists and are accepting new patients.

When you register you will also be asked to fill in a medical questionaire. This is because it can take a considerable time for us to receive your medical records. You will be asked to sign this form to confirm that the details are correct.
