New Patient Registration

Patients are free join any practice at Minden they wish. If no preference is expressed, then practices take it in turns to accept new patients on a monthly basis and you will be assigned appropriately. Please ask at our reception for a registration form. All practices currently have open lists and are accepting new patients.

Practice Area

We serve a geographic area comprising of mainly central Bury, but we accept patients from a wider area. As a rough guide, our Practice boundary extends to the following areas, although reception staff will be able to give you more detailed information if you are unsure:

North: Encompassing all of Tottington and Greenmount, with the northern-most boundary running along Kay Brow and Peel Brow in Ramsbottom.

East: Extends along Rochdale Old Road as far as Bamford Road in Heywood, then extending along Pilsworth Road to encompass the Summit area.

South: Extending along Hollins Lane, down Hollins Brow and then along to Spring Lane in Radcliffe.

West: Extending along Bolton Road in Radcliffe up to the junction of Turks Road and Countess Lane. Up Bradley Fold Road to encompass Ainsworth Village.

Pre-Registration with the practice

If you wish to pre-register, click on the link below to open the form. When you have completed all of the details, click on the 'Send' button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill in a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
